Important Notes and Reminders

The Personnel Action Form (PAF) is used anytime there is a change to an employee’s status. This includes new hire, termination, resignation, retirement, change in FTE/schedule, change in assignment or location, etc. Final, complete and approved PA forms must be obtained prior to any offer of employment or change to employment terms.

  • Hiring Manager is responsible for ensuring completion of this form.
  • For Certified Positions: Alli Fish can assist with questions about certified positions/employees
  • For Civil Service Positions: Loren Iorio can assist with questions about Civil Service positions/employees.
  • Budget codes can be obtained from budget development reports or WinCap for the prior incumbent of the position. If there are questions regarding budget codes, please contact Kathi Lewis (Chief Financial Officer). If there are more than three codes, attach a separate sheet.
  • When determining salary, remember that collective bargaining agreements and civil service often dictate starting salaries. If you are potentially looking at a salary outside of what is dictated by the Collective Bargaining agreement, be sure to discuss with your Assistant Superintendent and alert Human Resources. To reiterate – no offers can be made until which time the PA-form has been approved by all parties and accepted for the BOE agenda.
  • When an employee is changing positions, appointment to the new position should be accompanied by a resignation letter from their current position.
  • Once a hiring manager has completed the form, it can be printed and signed by the hiring manager. The hiring manager should then bring the form Human Resources and either Alli Fish or Loren Iorio will bring to their Assistant Superintendent for approval. The Human Resources designee (based on the class of employee) will work to gather any additional information, final approvals from the business office and placement on the Board agenda for action.
  • Once all approvals have been received and it has been accepted for the board agenda, a completed copy will be forwarded back to you. No offers for employment should happen until after you have received the completed form.
