Requests for Leave – Extenuating Circumstances
Directions
In some instances, employees may need to request leave time that falls outside of what has been contractually negotiated and/or is available to the employee as a part of their accruals.
Requests for unpaid leave generally must be approved by the Board of Education. This means that if an employee is seeking to take leave time that does not comply with their collective bargaining agreement between HFM BOCES and their association, the Request For Leave In Extenuating Circumstances Form. All other requests for leave must be made through Frontline.

Employee has a conversation with their supervisor that necessitates the need to take leave outside of what has been contractually negotiated.

Supervisor provides the Request for Leave In Extenuating Circumstances Form to employee.

Employee completes form and must also include an explanation of the circumstances that have necessitated the request, including why they do not have leave accruals available. Any forms submitted without this rationale will be returned.

Employee returns form to their supervisor for approval.

Supervisor provides form to the Assistant Superintendent that oversees the employee’s program/department.

Supervisor forwards form to Dr. Aaron Bochniak to obtain District Superintendent approval, BOE approval, and adjustment of employee’s accruals.

Approved forms will be returned to the employee and their supervisor.
